Reporting an injury

All injuries to players and volunteers occurring during a Little League activity should be reported to the Safety Officer. Here is how:

  1. Download the Little League Injury Report .
  2. Complete the injury report online.
  3. Submit it to our League Safety Officer at [email protected] as soon as possible.

In general, an injury report should be completed when an incident occurs requiring use of the first aid kit or a player sits out of play following the incident.

Concussion and Sudden Cardiac Arrest Compliance

To comply with the Zackery Lystedt Law (HB 1824) and the Sudden Cardiac Arrest Awareness Act (SB 5083), all players and parents must review the Concussion Information Form and Sudden Cardiac Awareness Information Form, then sign an acknowledgment form. The form may be submitted during one of the document turn-in dates or emailed to [email protected].

Sudden Cardiac Arrest Law

Sudden cardiac arrest is a leading cause of death for youth athletes. On July 24, 2015, Washington State enacted SB 5083, the Sudden Cardiac Arrest (SCA) Awareness Act, to increase awareness among youth athletes, families, and coaches.

To comply with the Act, the following is required:

  1. PARENTS/PLAYERS – Each year, parents/guardians and players must review an online pamphlet on sudden cardiac arrest. The compliance form is included during registration or can be downloaded here. Signed forms may be submitted at a document turn-in event or emailed to [email protected].
  2. ON-FIELD VOLUNTEERS – Every three years, volunteers must complete an online SCA prevention course at https://nfhslearn.com/courses/61032 and submit their completion certificate to [email protected].

Zackery Lystedt Law

The Zackery Lystedt Law requires concussion education for youth athletes.

Parents and players must review the Concussion Information Form during registration.

Managers must review the Coaches Concussion Information. All On-Field Volunteers are required to review this material.
